Registration Of Birth

1. Home Birth

a) The following Forms need to be submitted by applicant for registration and issuance of Birth certificate in case of Home birth:-

  1. Date of Birth certificate request form.
  2. Form No.7.
  3. Birth report Form.
  4. Form of Name Inclusion of child.

b) Document required for Birth Registration and issuance of Birth certificate within 1 year in case of Home birth:-

  1. Injection card Xerox.
  2. Ladies maternity card.
  3. Ration card/Aadhaar card.
  4. Home tax copy.
  5. Ward member letter/Certificate.

c) Document required for Birth Registration and issuance of non availability Birth certificate after 1 year in case of Home birth:-

  1. Injection card Xerox.
  2. Ration card/Aadhaar card.
  3. Home tax copy.
  4. Affidavit on 100 Rs. Stamp Paper.
  5. Intrusive Residential Interference

2. Hospital Birth

The following Forms need to be submitted by applicant for registration and issuance of Birth certificate in case of Hospital birth:-

  1.  Date of Birth certificate request form.

Searching fees for Birth entry per year Rs. 2/-
Issue for non availability Certificate for Birth Rs. 2/-
Late Fee For Birth Registration / Certificatefees
Inclusion of child name up to 15 years from the date of occurrence of Birth Rs. 5/-
Late Registration Fee after 21 days from date of occurrence of Birth Rs. 5/-
Late Registration Fee after court order Rs. 10/-

Registration Of Marriage

1. The work of regular registration of Marriage started from 02 May 2008 onwards.

2. For registration of marriage Rs.100/- are being charge for cost of marriage Registration form and Rs. 50/- as marriage registration fees, Rs. 100/- are being charged as late fees for registration on marriage from 90 days to 1 years period and Rs. 200/- are charged as late fees for registration of marriage from 1 years onwards

The following Form need to be submitted by applicant for registration and issuance of Marriage certificate

Registration of Death

1. Home Death

a) The following Forms need to be submitted by applicant for registration and issuance of Death certificate in case of Home death:-

  1. Death registration certificate request form.
  2. Form No.8.
  3. Death report Form.
  4. Form No. 4A.

b) Document required for Death Registration and issuance of Death certificate within 1 year in case of Home death:-

  1. Doctor death certificate of cause of death.
  2. Copy of Cremation Pass.
  3. Ration card/Aadhaar card.
  4. Tax Receipt.
  5. Bank Pass Book Xerox or I.D. Proof of the deceased person.
  6. Ward Member certificate.

c) Document required for Death Registration and issuance of non availability Death certificate after 1 year in case of Home death:-

  1. Doctor death certificate of cause of death.
  2. Copy of Cremation Pass.
  3. Ration card/Aadhaar card.
  4.  Tax Receipt.
  5.  Bank Pass Book Xerox or I.D. Proof of the deceased person.
  6. Ward Member certificate.
  7. Affidavit on 100 Rs. Stamp paper.

2. Hospital Death

3. Accident/Suicide

a) The following Forms need to be submitted by applicant for registration and issuance of Death certificate in case of Accident/Suicide:-

b) Document required for Death Registration and issuance of Death certificate within 1 year in case of Accident/Suicide:-

    1. Doctor death certificate of cause of death.
    2. Copy of Cremation Pass.
    3. Ration card/Aadhaar card.
    4. Tax Receipt.
    5. Bank Pass Book Xerox or I.D. Proof of the deceased person.
    6. Ward Member certificate.
    7. Police Panchnama.
    8. Postmortem report.

c) Document required for Death Registration and issuance of non availability Death certificate after 1 year in case of Accident/Suicide:-

    1. Doctor death certificate of cause of death.
    2. Copy of Cremation Pass.
    3. Ration card/Aadhaar card.
    4. Tax Pawati.
    5. Bank Pass Book Xerox or I.D. Proof of the deceased person.
    6. Ward Member certificate.
    7. Affidavit on 100 Rs. Stamp paper.

3. Who is responsible for giving the information to the Registrar?

Residents/Public of Cantonment Board Dehuroad has to provide necessary information for Registration

4. Issuance of certificates

Certificates are issued online after proper verification of documents.

5. Correction for erroneous entries

Correction of erogenous entries done at Cantonment Board Dehuroad.
